Complete our referral form with client details, needs assessment, and current supports. Include any relevant reports or assessments.
We review the referral within 2 business days and contact you to discuss the client's needs and our capacity to support them.
If we can support the client, we arrange an initial meeting to discuss their needs, preferences, and service options.
Once agreements are in place, services begin with regular communication back to the referring professional.
